Federal prosecutors estimate fraud against 14 Medicaid programs alone could total $9 billion, while the notorious Feeding Our Future scandal accounted for $300 million in federal meal program theft. The committee found that total losses across childcare assistance and nutrition programs exceed previous estimates by significant margins.

Investigators identified a recurring criminal business model exploiting low barriers to entry, shell companies, and kickback schemes to enroll recipients.
